Information

St. Lawrence Health Alliance, Inc. opened the doors of a new building at 3 Lyon Place, Ogdensburg, New York in August of 1995. The 36,000 sq ft Helen & Stanley McCadam Medical Office Building physically connects the Richard E. Winter Cancer Treatment Center to Claxton-Hepburn Medical Center and offers convenient access for patients to Medical Center services.

Appointments

· Schedule by calling listed physician number and speak with receptionists; it is generally a good idea to have your insurance card when calling so information can be collected prior to visit.

· Walk-ins are usually accepted if you are willing to wait, but always a good idea to call first.

· To cancel an appointment, it is requested you call 24 hours in advance.


Billing

· MasterCard and Visa are accepted

· Most insurances accepted

· Self-pay at time of visit unless other arrangements made

· Unpaid balance due within 30 days

· Fee information/special payment plans upon request


Emergency/After Hours Care

· Dial regular number and answering service will give instructions


Insurance procedures

· All insurance will be billed as a courtesy to the patient

· Different physicians accept different insurances


Medical records

· To obtain medical records:

-Sign a medical records release form and send to physician's office
-Indicate physician who is to receive record
-In some cases there may be a charge

New Patients

· Please bring your insurance card with you for your first office visit. If your insurance has copay, please be prepared to pay at the time of service.

· Some physicians require you to bring in prescription bottles of any medications you are taking

· New Patient Information Forms are required to be filled out and signed.

Office Hours

· Regular SLHA hours are 8:00 a.m. to 5:00 p.m.


Prescription Policy

· There is a 24 hour period for a call in prescription request.


Special Services (X-ray, EKG, Lab, Women's Services)

The convenience of an office complex that is physically connected to the hospital is extremely advantageous, as well as the availability of the diagnostic services right here in our building.

· Radiology and laboratory services are located on the second floor.

· Hepburn Health Center, a hospital clinic, provides pediatric, adolescent, adult and geriatric primary care.

· The Connection for Women, Claxton Women's Health Services, provides diagnostic ultrasound, mammography, bone densitomety screening, and massage therapy.
